Structure
The ACE Wellness Program is designed for individuals without a primary care provider who require ongoing monitoring and support for chronic health conditions. The program includes 6 personalized 20-minute visits over a 12-month period, available virtually or in person based on your needs.
Useage
Visits may also be used for episodic or acute care concerns throughout the year, including conditions such as sore throats, urinary tract infections, skin infections, colds, and other minor illnesses that can safely be assessed and managed in a clinic setting.
Program Exclusions
The ACE Wellness Program does not include IV Hydration Therapy, iron infusions, medical forms or documentation, or other services listed separately under the Prices & Plans section.
Appointments are not transferrable to anyone else.
Additional visits beyond the 6 included visits per 12-month period are available at a rate of $60 per 20-minute appointment.
