Terms and Conditions

Terms of Use Agreement

The user agrees that they may access the 'Website' from ACE Health and Wellness Inc. (the Owner), including all written materials displayed or made available through the website, such as articles, text, photographs, images, illustrations, audio clips, video clips, and computer software. (the "Content").  By accessing and using the Website, you acknowledge and agree to be bound by the Terms and Conditions of this agreement.  You must not access the Website if you do not accept these Terms and Conditions.  The Owner may revise this Agreement at any time by updating this posting.  Use of the Website after such changes are posted will signify your acceptance of these revised terms.

Medical Emergencies

If you are experiencing a medical emergency, CALL 911 or go to your nearest emergency department.  This Website is for online booking of appointments for services mentioned on the Services page.  Under no circumstances are you to self-diagnose or treat yourself based on anything you read or have seen on this Website.

Medical Advice

The 'Content' of this website is for educational and information purposes only.  This Website is not a substitute for medical advice, diagnosis, treatment, or care and should not be treated as such.  Always consult a medical professional and follow their treatment plan.

Appointment Cancellation ​

If you need to cancel or reschedule an appointment, please notify us with at least 24 hours' notice. You can do this on the booking site or by email at info@acehealthandwellness.ca. If it is within 24 hours, please call us at 613-979-1345. Failure to cancel outside of 24 hours will result in a $30 cancellation fee. No fees will be charged if ACE Health and Wellness Inc. cancel the appointment.

Fees and Payment 

We are transparent about our fees and services.  Fees are listed on the website and are subject to change.  Fees and services will be reviewed annually in April, and changes will be announced 3 months before implementation.  Payment is due after services are performed, unless otherwise agreed upon for longer-term contracts.  Payments can be accepted by cash, cheque, credit card or e-transfer to acehealth23@gmail.com.  Official receipts, including HST numbers and company information, will be provided to enable clients to submit them for reimbursement.  We do not directly bill any third-party or insurance companies.  Nurse Practitioners cannot bill OHIP when this policy was written, and updates to the website regarding changes in legislation will be communicated promptly, as well as fee changes. Failure to remit payment will result in being unable to book future appointments.

No-Show

A fee of $30 will be applied to a no-show appointment without a foreseeable reason.  After 2 no-show appointments, the client cannot book without ACE Health and Wellness staff permission.

Prescriptions

You agree that any prescription you receive from ACE Health and Wellness Inc. is for the purpose of the person for whom the prescription is written.  Any new prescriptions require a complete initial assessment before being delivered.  Prescription renewals not initially written by an ACE Health and Wellness Inc. professional must have a consultation before being renewed.  Prescription renewals of medications initially prescribed by an ACE Health and Wellness Inc. professional will be assessed on a case-by-case basis, and a fee of $20 will apply if no assessment is required. We will not be prescribing controlled substances.

Safety on the Home Visits

The safety of our clients and our staff is our top priority, as we will potentially be treating you in your home. If at any time during a home care visit a client feels unsafe or uncomfortable, the client may ask the staff to leave, and no fees will be applied to the visit. If at any time during a home care visit a staff member feels unsafe, they have the right to leave immediately, and the full fee will still be applied to the client.

Refusal to Treat

It is the right and duty of the provider to use their medical skills and judgement to assess, treat and implement care plans for a patient.  It is the provider's right to refuse to treat if there is a conflict of interest, safety concerns or for moral or legal reasons. If a patient threatens, intimidates, or makes the provider feel unsafe, the provider has the right to refuse to treat the patient if it is safe. A complete fee will be applied to the patient's visit.

Availability of Staff

The hours of service and dates will be posted on the website and updated regularly. Emails and phone calls will be responded to within 48 hours, except on weekends and holidays. At this time, there is no physical office, therefore any requests to meet in person will be arranged at the client's home.

Chronic Clients

Any clients taken on in the membership plan for chronic patients is up to the discretion of the NP, as there is limited availability.  Memberships are$500 a year and allow the client 6 visits in a calendar year. Additional visits are $75 each.

Clients Rights to Access Information

Clients of ACE Health and Wellness Inc. have the right to access their own records upon request to info@acehealthandwellness.ca and to request any changes be made to their personal information.

The most up-to-date policies are in hard copy at the main office of ACE Health and Wellness Inc., and a copy can be provided upon written request.
