Prices and Plans

Nurse Practitioners are currently unable to bill OHIP for their services, which means we operate on a fee-for-service basis. Some services provided by Registered Nurses may be covered by your private insurance. We will issue receipts and assist you with any necessary documentation to help facilitate the reimbursement process. Please note that we do not bill insurance companies directly, and all services must be paid for at the time they are rendered. Direct patient care services are exempt from HST. Registered Nurse In-Home Assessment

$50 per half hour

The Registered Nurse will visit the patient's home to provide nursing services. An additional $0.60 per km (HST included) will be charged from the office. We are currently serving the Russell, Embrun, and surrounding areas.

Registered Nurse Virtual Assessment

$40 per half hour

Assessments, education, resources or follow-ups are conducted via secure video or phone calls from the comfort of your own home.

Respite Care

$80 per hour

Keeping your loved ones at home is essential for their quality of life. However, caregiver burnout occurs when we fail to give ourselves time to rest and rejuvenate. Respite care is only booked after consultation with the RN. Don't hesitate to Contact Us for more information.

Nurse Practitioner Initial Assessment

$150

The Nurse Practitioner will conduct a virtual assessment via secure video call or telephone to gather medical information and determine if further testing is needed to formulate a diagnosis and develop a treatment plan.

Nurse Practitioner Follow-Up

$75 for half hour

The Nurse Practitioner will follow-up with any treatment plans, diagnostics or relay results via secure video or voice calling.

Annual Check-Up

$250

The annual check-up includes a thorough physical assessment, a review of your health history, including medications, vaccinations, and any current concerns. It also encompasses testing and screening, such as bloodwork, preventive care, and follow-up with a Nurse Practitioner.

Form Completion

$80

Forms that do not require physical examinations and are less than 5 pages. This includes picking up and dropping off the forms and submitting them as needed.

Ministry of Transportation Medical Form

$250

This includes physical assessment, completing the forms, and submitting them to the MTO. This can be done at home or at your workplace. Inquire for details.

Tax Forms T2201

$250

This involves a thorough review of medical history, medications, disability, a physical examination as needed, and completing necessary forms to be submitted to the CRA. Assistance is given to obtain appropriate supportive medical documentation.

Episodic Visits

$100

Episodic visits are in-home visits by the RN for assessment. If a diagnosis is required, an NP consult will be an additional $30, and if a prescription is needed, an additional $20 will be charged. Mileage is charged at $0.60/km (including HST).

Diabetic Screening

$200

Diabetic screening includes a comprehensive physical examination, lab work, and follow-up with the Nurse Practitioner.
